The Health Information Management (HIM) Department at Nationwide Children's Hospital provides patients, patient families and healthcare professionals with medical records such as nurse’s notes, lab tests, reports and treatment plans.
Medical Record Request Process
To request your medical records, you may submit your request by MyChart, email, fax, mail or in-person.
By MyChart:
Simply log in to your child’s MyChart account to access their medical information. You can view, print or download information to your computer. Should you need to formally request records, you can do so directly through MyChart via the Document Center. Have a MyChart question? Please call (614) 355-0852.
By email:
Your completed Medical Record Request Form can be emailed to MedicalRecordRequests@NationwideChildrens.org.
By fax: Your completed Medical Record Request Form can be faxed to (614) 355-0797.
In person (for urgent requests only):
For your convenience, authorization forms are available in all clinic locations. Once you’ve completed the form, please have the clinic staff forward it to the HIM department.
